Reduce your paper waste and register your guest online prior to the appointment. Guests authorized online no longer need a printed out a letter of permission. By making the appointment online, you are explicitly giving this guest permission to shop under your organization. You and your organization will be held responsible for your guest’s behavior and use of materials taken from the MFTA warehouse. If you are unable to register your guest online, or if your guest has already made an appointment by calling the office, he or she may shop for your organization only if authorized with a completed and signed permission form linked below. Forms must be filled out completely and be signed by an authorized (“Primary” or “Secondary”) contact. Guest forms for schools can only be signed by the principal.
